Cory Monteith Died from a Lethal Combination of Heroin and Alcohol

Glee star Cory Monteith reportedly died from a lethal combination of heroin and alcohol.
TMZ reports that the British Columbia Coroners Service has conducted an autopsy on the 31-year-old star, who was found dead in his Vancouver hotel room on Saturday. According to a source close to the investigation, toxicology analysis revealed that Monteith died from a "mixed drug toxicity, involving heroin and alcohol."

UPDATE: The British Columbia Coroners Service has released the following statement about Monteith's cause of death:
"There is no evidence to suggest Mr. Monteith's death was anything other than a most-tragic accident. Mr. Monteith's family has been made aware of the circumstances surrounding the death. On behalf of family members, the BC Coroners Service asks that the media respect their privacy at this difficult time."
Monteith had struggled with substance abuse in the past, recently admitting himself to a rehab facility last spring. He completed the rehab program in April and seemed to be in good spirits as he prepared to begin working on Glee's upcoming fifth season. In a statement about the star's passing, Glee producers said:
“We are deeply saddened by this tragic news. Cory was an exceptional talent and an even more exceptional person. He was a true joy to work with and we will all miss him tremendously. Our thoughts and prayers are with his family and loved ones.” 
Girlfriend and co-star Lea Michele has not spoken publicly about Monteith's passing, but her rep issued a statement on her behalf on Sunday, saying, “We ask that everyone kindly respect Lea‘s privacy during this devastating time."
